We are now offering our seminars ONLINE! The Online Seminar contains the same in-depth information provided at our live seminars and is conveniently available whenever and wherever you want. There is no cost or obligation, and upon completion you will have all the information needed to decide if weight loss surgery is right for you.

Every patient will need to complete a "Patient Information and Health History Packet".  Click on the "forms" below.  Then download the packet.  You can save the packet to your documents or desktop, complete it, and email it back to our office coordinator at sgary@bhsi.com or you can print the packet, complete it, then return it to us via mail, fax, or personal delivery at the seminar or our office.  Thanks.  If you need a packet by mail, call Sabrina at 859-263-2022.
